Making An Appointment

To become a patient at LifeQuest, please arrange a referral from your family physician, gynecologist or other fertility specialist. Referrals can be faxed to LifeQuest at 416 506-0600, or mailed to LifeQuest 655 Bay St. 18th floor Toronto, ON M5G 2K4.

Hours

Our hours are: 8 a.m. to 4 p.m. We operate daily, year-round, with the exception of a two-week break in late December. Please call the clinic for specific dates.

LifeQuest's downtown clinic is centrally located at 655 Bay Street, 18th floor, in downtown Toronto on the north-east corner of Bay and Elm, between Dundas and Gerrard Streets.
